Animal Adoption Agreement


Adoptable Animal Information.  

The Woodhaven and Trenton Animal Shelter does adopt out homeless and unwanted animals.  Open adoption hours are Monday thru Friday 10am to 3pm.  Appointments are also available 8-10 am, 3-5pm and weekends. 
The Animal Shelter adopts out dogs, cats and sometimes small animals. 
 The dogs and cats that are up for adoptions and are 4 months old or older, will be tested, de-wormed, given proper vaccines, microchipped and altered.  Adoption fee for these animals are $100 cash or check.  
Adoptable dogs and cats that are under 4 months of age and that are to young to be altered will go out on a Spay/Neuter Contract.  These animals must be at least 8 weeks in age.  They will be de-wormed, tested (depending on its age), given proper vaccines and microchipped (depending on its age).  Adoption fee is $100 cash or check, but the adopter will get $50 back once they have the said animal altered per the Spay/Neuter Contract. 

Occasionally the Animal Shelter will have other small animals up for adoption.  To ensure that these animals go to  loving homes, the adoption fee is $25.00 per animal. The small animals may range from rabbits, birds, hamsters, gerbils, guinea pigs, farm animals and reptiles. 

Potential Adopters will need to first fill out the Animal Adoption Application, See below downloadable document.  Once the Animal Adoption Application is approved, the adopter will fill out and sign the Adoption Agreement.  If the animal is not altered, the adopter will also need to fill out and sign the Spay/Neuter Contract.

Animal Adoption Agreement Information

Once the Animal Adoption Application is approved, That animal's Adoption Agreement will be filled out by the  Animal Shelter staff and given to adopter to read, agree and sign.  The Adoption Agreement will be required for every animal that is adopted out of the Woodhaven and Trenton Animal Shelter. 

The Adoption Agreement will state the name, address and phone number of the adopter.  The Adoption Agreement will also state the animal general information.  

The adopter will need to read, agree and sign the Adoption Agreement, which states the following: 

1). To provide food, water, proper shelter, city pet license, exercise, regular veterinarian care and immediate medical attention or follow up care for a currently treated condition, if the animal becomes sick or injured.  
2).  I will not allow the animal to run at large or violate any City or County ordinance and laws or regulations of the State of Michigan, including but not limited to those regarding animal cruelty or licensing. 
3).  I understand that I will pay a non-refundable $100 cash or check adoption fee for this animal.  
4). I understand that I have 7 days to bring the animal back for any reason.  After 7 days, I will be charged a owner surrender fee of $40 cash or check or the Animal Shelter may refuse to take the animal back .  
5).  I understand that the Cities of Woodhaven and Trenton Animal Shelter strives to place only healthy animals.  I also understand that the Animal Shelter cannot guarantee the health, fitness, age, the sex (including but not limited to pregnancy) or temperament of the animal and whether the animal is housebroken.  
6). I understand that the said animal was held for the proper amount of stray time in accordance with the State of Michigan or was legally signed over to the Woodhaven and Trenton Animal Shelter. 
7).  I understand that I will not hold the Animal Shelter, Cities of Woodhaven and Trenton, and all its employees, volunteers or representatives responsible for any damages that may result for the said animal. 
8).  In the event that legal action by the Cities of Woodhaven and Trenton Animal Shelter is necessary because I failed to comply with the terms of this agreement and expenses and/or court and attorney fees are incurred, I agree to pay for those expenses and fees incurred by the Animal Shelter.  I also understand that I will then be place in the "Do Not Adopt List".  

Dog and Cat Spay/Neuter Contract Information

Dogs and cats not spayed or neutered must have a Spay/Neuter Contract signed by the adopter at the time of adoption.  A $50 cash or check refundable deposit will taken at the time of adoption.  The adopter will be given a date that the said animal must be altered by.  The adopter may take the animal to a license veterinarian of their choice.  
The Spay/Neuter Contract is in accordance with the State of Michigan Law - MCL PA 287.331.
The State of Michigan requires all dogs, cats and ferrets over the age of 6 months that are adopted from any Michigan Animal Shelter to be spayed or neutered by a license veterinarian within 30 days.  All animal under 6 months of age must be spayed or neutered within 30 days after they become 6 months of age.  Failure to do so will result in State and/or local fines and possible forfeiture of the animal to be returned back to the Animal Shelter.  

The Spay/Neuter Contract will be filled out by the Animal Shelter staff and will state the adopters name, address and phone number.  The Spay/Neuter Contract will also state the animal general information and the date that the animal must be altered by.  
The adopter will need to read, agree and sign the Spay/Neuter Contract, which states the following: 
1). I understand that I will pay the Cities of Woodhaven and Trenton Animal Shelter a Spay/neuter deposit of $50 cash or check.  This $50 deposit is refundable upon verification of spay/neuter by a licensed veterinarian of my choice.  
2).  If for any reasons the animal cannot be spayed or neutered by the below described date, my veterinarian will give written verification for the reason why the animal cannot be spayed or neutered.  The animal must then be reevaluated after 14 days, and within 7 days of the health report. 
3).   I agree to have the spay/neuter procedure done on or before the following date by a licensed veterinarian of my choice.  I will bring the spay/neuter certificate and a copy of the receipt to the Animal Shelter by the following date.   The date is:________________.
4). I understand that a breach of this contract, by not sterilizing this animal by the above date, will result in a $100 liquidation damage cost and/or forfeiture of the said animal.  I also understand that I will lose my $50 spay/neuter deposit and will be place on a "Do not adopt" list. 
5).  I understand that refunds are issued by the City of Woodhaven.  The refund may take at least 30 days and will be mailed to you.
